3Eligibility Requirements
Eligibility for billing the A255 code is determined by the following criteria derived from OHIP guidelines:
- It must be rendered following a written request from a referring physician, nurse practitioner, or dental surgeon in the context of an insured dental procedure within a hospital.
- The service must meet the same consultation frequency limits, meaning only one service per two consecutive 12-month periods for the same patient, physician, and diagnosis. An exception is granted when a consultation is for a distinctly different diagnosis in the same period.
- A255 can be billed virtually only if conducted via video, not by telephone, as outlined in Appendix J for eligible comprehensive virtual care services.
Should these requirements not be met, fees may be reduced to a lesser assessment rate as per General Preamble GP16.
