1. Type 2 diabetes mellitus: Discussed importance of maintaining HbA1c below 7%. Advised on dietary modifications including high-fibre, low-glycemic foods, avoidance of sugary drinks, and limiting salt intake to about 1 teaspoon per day. Encouraged regular physical activity, including walking and light strength training twice a week. Emphasized the importance of home blood glucose monitoring and keeping a log. Referral to dietitian for personalized dietary guidance.
2. Essential hypertension: Discussed the need to keep blood pressure under 130/80 mmHg. Advised on sodium restriction, reading food labels, and using herbs for flavor instead of salt. Reinforced the role of lifestyle modification in blood pressure control.
3. Dyslipidemia: Discussed the goal of keeping LDL cholesterol under 2.0 mmol/L. Reinforced dietary and lifestyle changes to improve lipid profile.
