Ms. Taylor was seen today regarding surgical follow-up for breast cancer. She had surgery. Since I saw her last, she has had treatment as follows: Objective: On examination today, there is no mention of hair thinning, constipation, or feeling unusually cold being absent. Investigations: Laboratory investigations performed include thyroid stimulating hormone, which is elevated at 7.8, free thyroxine, which is low-normal, and anti-thyroid peroxidase antibodies, which are positive. A vitamin D panel was also performed, revealing mild deficiency. Assessment and Plan: Assessment/Plan: I have explained to the patient that we typically discharge patients from the Regional Breast Center after their one-year follow-up visit for breast cancer due to the limitations of the hospital clinic space. We have recommended that she get her routine follow-up with, but I would be happy to see her again at your request if there are changes or concerns. In terms of longer-term follow-up, I would recommend: - Monthly breast self-examination. - Six-monthly examination with her family physician for the first 5 years and annually after that (for physical examination of the breast, chest wall, and axilla bilaterally). - Annual mammograms unless she has had a bilateral mastectomy.
