Cardiology Follow-Up Consult Concise

I had the pleasure of reviewing Mr. Smith today. 

Diagnoses:
- Shortness of breath (R06.02)
- Orthopnea (R06.01)
- Localized swelling, mass and lump, lower limb, bilateral (R22.43)
- Palpitations (R00.2)
- Hypertension, diabetes mellitus

Interval History:
Mr. Smith reports progressive dyspnea over the past two weeks, most pronounced with exertion such as walking short distances and ascending stairs. He experiences orthopnea, requiring three pillows to sleep due to coughing and difficulty breathing when lying flat. He reports bilateral lower extremity edema that worsens by the end of the day, describing his legs as puffy and tight. He notes an unintentional weight gain of approximately six pounds over the last ten days without changes in dietary habits. He denies chest pain but reports intermittent palpitations described as a sensation of his heart racing. He continues to take his antihypertensive and antidiabetic medications daily without missed doses.

Mr. Smith presents with progressive dyspnea, orthopnea, bilateral lower extremity edema, and intermittent palpitations. His symptoms are suggestive of a possible underlying cardiac or fluid overload condition requiring further evaluation.
