Subjective:
Purpose of the Visit:
Ms. Parker presents for evaluation of persistent fatigue and associated systemic symptoms.

Chief Complaint (CC):
Ms. Parker reports persistent fatigue over the last few months, which has impacted her ability to work and care for her children.

Objective:
Medically Significant Findings:
Ms. Parker has a family history of autoimmune thyroid disease, specifically Hashimoto's thyroiditis in her mother. She reports weight gain, irregular menstrual cycles, morning joint stiffness, hair thinning, constipation, and increased sensitivity to cold. Laboratory findings within the last week reveal elevated thyroid stimulating hormone at 7.8, low-normal free thyroxine, and positive anti-thyroid peroxidase antibodies, consistent with autoimmune hypothyroidism. She is also noted to have mild vitamin D deficiency. No mention of hair thinning, constipation, or feeling unusually cold being absent.

Procedure performed:
- Laboratory evaluation of thyroid function and vitamin D status
- Prescription of levothyroxine 50 micrograms daily
- Recommendation for vitamin D supplementation at 2000 IU daily

Assessment and Plan:
- Problem 1: Autoimmune hypothyroidism, E06.3. Ms. Parker is diagnosed with autoimmune hypothyroidism based on clinical symptoms and laboratory findings. Initiation of levothyroxine at 50 micrograms daily is recommended, with repeat thyroid function testing in six weeks to assess response and adjust dosage as needed. She is advised to take levothyroxine on an empty stomach, at least 30 minutes before breakfast, and to avoid concurrent intake with calcium or iron supplements. If joint stiffness persists, referral to rheumatology will be considered.
- Problem 2: Vitamin D deficiency, E55.9. Ms. Parker is advised to take vitamin D supplementation at 2000 IU daily and to increase sunlight exposure as feasible. Reassessment of vitamin D status is planned in three months.
