Clinical Findings
[Describe any clinical findings. Omit subheadings if not discussed. Extraoral / TMJ, Facial symmetry, Lymph nodes TMJ: clicking, tenderness, deviation, ROM, Musculature tenderness (if noted) Intraoral Soft Tissue Exam Gingiva (generalized/localized inflammation) Periodontal Assessment - Generalized / localized probing depths, Bleeding on probing (BOP), Recession areas, Furcation involvement, Mobility (by tooth number if present), Periodontal diagnosis (health / gingivitis / periodontitis stage & grade if applicable) Hard Tissue Exam (Tooth-by-Tooth Findings) Document clearly by tooth number, using concise but specific language: Existing restorations (type and condition), Caries (location, depth estimation), Fractures, cracks, wear facets, Occlusal issues, open margins / overhangs, Missing teeth, Example formatting (no bullet): 18DB existing composite restoration, intact 17MO caries into dentin, pulpal proximity, asymptomatic 16RCT with amalgam core; PARL present but showing signs of healing 15–14RCT with splinted PFM crowns; bulbous margins; 15 PARL noted 24 pevious RCT with screw post and composite buildup; fractured soon after placement; recurrent caries noted today Q[Quadrant Number] normal to percussion. Q[Quadrant Number] normal to palpation except [specific tooth/area]. Q[Quadrant Number] normal to mobility/probing. Q[Quadrant Number] negative to ice stick except [specific tooth/area]. Q[Quadrant Number] negative to endo ice except [specific tooth/area]. Updated BW/PA/pan/CBCT w/ pt permission (if applicable), radiographic Findings (Periapical radiolucencies - size, change over time, Bone levels, decal/caries. comparison to previous images (date if known), Photos/scan impressions taken]
Diagnosis
[State the diagnosis, including affected tooth/teeth number, condition (periodontal eg perio abscess. Biomechanical eg carious or non-carious fracture into enamel/dentin/pulpal exposure. Pulpal e.g., reversible pulpitis, irreversible pulpitis, necrotic w/symptomatic apical periodontitis, necrotic w/ asymptomatic apical periodontitis, previous RCT, etc), and any contributing factors such as trauma. Each issue should be clearly identified and clinically reasoned. Occlusal or functional concerns Example: 17MO caries with pulpal proximity; currently asymptomatic 15–14 splinted PFM crowns with marginal discrepancy and PARL, symptomatic apical periodontitis 13 perio-endo lesion, localized 5mm probing along B 11 suspected VRF - poor to hopeless prognosis 24 fractured post-core with recurrent caries; guarded restorative prognosis ]
Prognosis and Recommendations
[Provide the prognosis for the affected tooth/teeth, including any challenges or risks associated with treatment. Outline the recommended treatment plan, including additional imaging eg PA, CBCT, pulp vitality testing, caries excavation to assess restorability, and/or procedures such as root canal therapy (RCT), core buildup, crown placement, or provisional restorations. Mention any referrals made and the rationale for the referral. Example: Tooth 24: Discussed removal of existing restoration and post to assess restorability. Reviewed potential need for crown lengthening, new post/core, crown, or extraction if non-restorable. ]
