Review of Systems

- Growth: Emma had appropriate weight gain until approximately two months of age, followed by a plateau in weight from three to four months. Mother reports Emma sometimes falls asleep during feeds and has not gained much weight recently.
- Diet: Emma is currently exclusively breastfed via direct nursing. The mother previously used a breast pump and bottle-fed but has not done so in the past month. There is uncertainty regarding the volume of milk Emma consumes per feed. The mother has a breast pump available at home.
- Stooling: Advised to monitor for at least 5 wet diapers per day and track stool frequency to assess hydration.
