Procedures:

Impression and Plan:

   - Recommendations: Patient to remain NPO. Intravenous fluids (lactated Ringer's) at 100 mL/hour. Abdominal ultrasound scheduled to evaluate gallbladder. Liver function tests (including ALT and bilirubin) ordered. Intramuscular injection for pain management administered with discussion of possible side effects (dizziness, nausea, constipation).

Patient Instructions:
Patient and family informed regarding the current condition and potential causes, including possible biliary pathology. Abdominal ultrasound and liver function tests have been ordered to further evaluate the gallbladder and liver.
