History of Present Illness:  
- The patient presents with a facial injury following a fall from an electric bicycle earlier today. The patient reports striking the face on the pavement after hitting a curb and being thrown over the handlebars. The patient was wearing a helmet at the time. The patient denies loss of consciousness or vomiting since the fall. Pain is localized to the facial region, with mild neck soreness noted, particularly with rotation. There is no history of prior similar injuries or facial trauma.

Physical Examination:

- The patient was alert and oriented.
Vitals: 
HEENT:  
- Facial examination showed no major cuts or swelling, and the nose was not deviated.
- A small abrasion was noted on the nose.
- Palpation of the nasal bridge revealed crepitus, suggesting a possible nasal fracture.
- Internal nasal examination showed no septal hematoma.
- The right eye exhibited a subconjunctival hemorrhage, but vision was intact, and intraocular pressures were normal at 14 bilaterally.
- There were no signs of hyphema.
- The teeth were examined and found to be undamaged.

- Examination of the neck revealed no major tenderness or swelling, and the patient exhibited full range of motion with mild soreness on rotation.

Clinical Impression:
The patient presented after a fall from an electric bicycle, landing face-first on the pavement. There was no loss of consciousness or vomiting. Examination revealed a small nasal abrasion, crepitus over the nasal bridge suggestive of a nasal fracture, and a right subconjunctival hemorrhage with preserved vision and normal intraocular pressures. No septal hematoma or dental injury was identified.

Differential Diagnosis:
- Nasal bone fracture
- Soft tissue facial injury
- Ocular injury (subconjunctival hemorrhage)

Assessment & Plan:
1. Facial trauma with suspected nasal fracture
- Treatment planned for Issue 1: Tetanus booster administered, pain medication prescribed, instructions given to avoid aggressive nose blowing and to apply ice to the area.
- Relevant referrals for Issue 1: Referred to otolaryngology (ENT) for further evaluation and management.

2. Right subconjunctival hemorrhage
- Treatment planned for Issue 2: Observation, as vision is intact and intraocular pressures are normal.
