Dear ,

Thank you for your referral of Riley, a 6-year-old male presenting with concerns regarding speech and academic development following tympanostomy tube surgery. Below is a summary of the assessment and plan.

IMPRESSION AND PLAN
Riley is a 6-year-old male presenting with persistent speech delay, academic difficulties (particularly in literacy), and distractibility.

1. Speech and Academic Development Delay: Ongoing, resulting in significant challenges with literacy, letter recognition, and attention span. Discussed lifestyle strategies including direct engagement during communication, frequent home practice, and use of teacher-provided strategies. Provided education on the importance of early intervention. Patient is on the waitlist for school-based speech therapy services. Explored the possibility of online speech therapy, though attention span may be a limiting factor.

2. Tympanostomy Tube Follow-Up: Post-surgical course has been uncomplicated with improved hearing and speech clarity. Will forward results of audiology testing once completed.

No ADHD diagnosis is suspected at this time.

FOLLOW-UP
I suggest continued communication with the school team to advocate for prioritization of speech therapy services and recommend exploring online therapy options as appropriate. Please forward audiology results when available. Thank you for involving me in the care of this patient.
