Physical Exam:
Patient looked well and was not in any acute distress. No mention of hair thinning, constipation, or feeling unusually cold being absent.

Investigations:
TSH is elevated at 7.8. Free T4 is low-normal. Anti-thyroid peroxidase antibodies are positive. Vitamin D panel shows mild deficiency.

Discussion Summary:
Ms. Taylor is diagnosed with hypothyroidism, likely of autoimmune origin, based on elevated thyroid stimulating hormone, low-normal free thyroxine, and positive anti-thyroid peroxidase antibodies. The clinical presentation of fatigue, weight gain, irregular menses, joint stiffness, hair thinning, constipation, and cold intolerance is consistent with this diagnosis. Mild vitamin D deficiency is also identified.

Ms. Taylor is advised to begin low-dose levothyroxine at 50 micrograms daily. The expected time to clinical improvement is discussed as six to eight weeks, with plans to recheck thyroid function tests in six weeks for dose adjustment. For joint stiffness, it is explained that this may be related to hypothyroidism, but rheumatology referral will be considered if symptoms persist. For vitamin D deficiency, supplementation with 2000 international units daily is recommended, with repeat testing in three months.
