- Mr. David is here for follow-up of diabetes management. He states he has been trying to keep his sugars under control, is eating better, and walking more. He reports no side effects from metformin. He has no known drug allergies. He tests his blood glucose at home about three times a week, usually in the morning. His younger brother was recently diagnosed with diabetes.
2. Hypertension:
- Mr. David is here for follow-up of hypertension management. He reports he is trying to keep his blood pressure under control. He takes ramipril daily and reports no side effects. He has no known drug allergies.
- This visit is a follow-up for diabetes and hypertension. We discussed available test results, that show recent laboratory and vital sign measurements. Investigation done to reassess control of diabetes and hypertension. We reviewed results with Mr. David.

- HbA1c: 7.2%
- LDL cholesterol: 2.4 mmol/L
- Triglycerides: 1.8 mmol/L

COUNSELING :

We discussed with the patients findings on exam, available and pertinent tests results and reports. We discussed with the patient significance of findings.
{Discussed healthy lifestyle, role of regular moderate physical activities. | Reviewed ABCDE of melanoma, advised to avoid sun exposure, to use protective clothing. | Discussed proper use of medical resources and importance of regular checkups, preventive health care visits, immunization.}
1. Type 2 diabetes mellitus: We discussed the importance of maintaining HbA1c under 7%. Mr. David was advised to continue dietary modifications focusing on high-fibre, low-glycemic foods, avoid sugary drinks, and to keep a log of home blood glucose readings. Referral to dietitian for personalized guidance was made. We discussed the significance of his brother's new diagnosis and reinforced risk factor management.
2. Hypertension: We discussed the importance of keeping blood pressure under 130/80 mmHg. Mr. David was advised to limit salt intake to about 1 teaspoon per day, read food labels for sodium content, and use herbs for flavoring instead of salt. He was encouraged to continue regular walking and to add light strength training twice a week if possible.
3. Dyslipidemia: We discussed the goal of keeping LDL cholesterol under 2.0 mmol/L and the importance of ongoing dietary and lifestyle modifications.
4. Overweight: Mr. David was encouraged to continue physical activity and dietary changes to support weight reduction.
MEDICATIONS:
1. Metformin (common side effects: gastrointestinal upset, diarrhea, metallic taste, vitamin B12 deficiency)
2. Ramipril (common side effects: cough, dizziness, hyperkalemia, angioedema)
