DIAGNOSIS:

CURRENT TREATMENT:
David is currently managed with metformin for diabetes and ramipril for hypertension. He reports taking both medications daily with no medication side effects. No chemotherapy, radiation, or other oncologic therapies are in use.

CLINICAL STATUS:
David presents for follow-up of diabetes and hypertension management. He reports adherence to dietary modifications and increased physical activity. Home blood glucose monitoring is performed approximately three times per week. There are no known allergies. On examination, blood pressure is 138/84 millimeters of mercury, weight is 84 kilograms, and body mass index is 28.3. Fasting glucose is 7.8 millimoles per liter, and hemoglobin A1c is 7.2 percent. Low-density lipoprotein cholesterol is 2.4 millimoles per liter, and triglycerides are 1.8 millimoles per liter. Heart sounds are normal, lungs are clear, and abdomen is soft with no abdominal tenderness. There is no ankle swelling. Foot sensation is intact, with no foot ulcers or skin issues. No pending laboratory or diagnostic test results are noted.

David is advised to continue current medications and lifestyle modifications. He is instructed to focus on a high-fiber, low-glycemic diet, avoid sugary drinks, and reduce sodium intake to approximately one teaspoon per day, including processed and restaurant foods.
