PAST MEDICAL HISTORY No relevant medical conditions or previous surgeries/hospitalizations reported. MEDICATIONS No current medications reported. SOCIAL HISTORY Fatigue affects her ability to work and care for her children. INVESTIGATIONS Laboratory investigations reveal an elevated thyroid-stimulating hormone (TSH) level of 7.8, a low-normal free thyroxine (T4) level, and positive anti-thyroid peroxidase (anti-TPO) antibodies, consistent with autoimmune hypothyroidism. A Vitamin D panel indicates mild deficiency. IMPRESSION AND PLAN Ms. Taylor presents with symptoms and laboratory findings consistent with autoimmune hypothyroidism and mild vitamin D deficiency. PLAN - Initiated levothyroxine 50 mcg daily, to be taken on an empty stomach, ideally 30 minutes before breakfast. - Thyroid function tests (TSH and free T4) to be rechecked in six weeks to evaluate response and adjust dosage if necessary. - Vitamin D supplementation at 2000 IU daily recommended, with rechecking of Vitamin D levels in three months. - Lifestyle modifications discussed, including maintaining a balanced diet, engaging in regular light physical activity, and ensuring adequate sunlight exposure. - Advised to avoid taking thyroid medication simultaneously with calcium or iron supplements. - Monitoring of joint stiffness; if symptoms persist, consider referral to rheumatology. - Follow-up appointment scheduled in six weeks to review thyroid function tests and assess treatment effectiveness.
