HPI:
The patient reports doing well overall. He monitors his blood pressure at home and notes a reading of 90/53 mmHg this morning. He also reports a lower blood pressure reading of 79/57 mmHg taken at the dentist's office yesterday. He denies experiencing dizziness with these low blood pressure readings. He does not report any breathing difficulties or shortness of breath during regular activities. He only experiences mild shortness of breath with sudden, intense exertion, such as rapidly ascending a mountainside, but is able to walk for 20 minutes, including up and down hills, without issue. He is able to walk up three flights of stairs without difficulty. He expresses no complaints regarding his current medication regimen.

ROS:
Cardiovascular – No dizziness when blood pressure drops to low levels. No complaints about the medications. Respiratory – No breathing issues or shortness of breath during regular activities.

Family History:
No family history provided.

History of heart failure. History of hypertension.
