HPI:
The patient is being followed for heart failure and hypertension. The patient reports that blood pressure readings at home are low, with a measurement of 90/53 mmHg this morning and a reading of 79/57 mmHg at the dentist yesterday. The patient denies dizziness or lightheadedness associated with these low blood pressure readings. The patient denies any breathing difficulties or shortness of breath during regular activities. The patient only experiences mild exertional dyspnea when transitioning abruptly from rest to rapid exercise, such as ascending a mountainside quickly, but is able to walk for 20 minutes, including up and down hills, without symptoms. The patient's spouse confirms the ability to walk up three flights of stairs without difficulty. The patient expresses no complaints regarding the current medication regimen.

ROS:
Cardiovascular – No dizziness when blood pressure drops to low levels. No chest pain or palpitations reported. Respiratory – No breathing issues or shortness of breath during regular activities. No cough or wheezing reported. Medications – No complaints about the medications.
