Dear Dr. Patel, Thank you for your referral of Ethan for pediatric consultation. He was evaluated in our office on May 13, 2025. REASON FOR REFERRAL: Dietary concerns and language delay. HISTORY OF PRESENT ILLNESS: Ethan is a 23-month-old male presenting for follow-up regarding dietary concerns and language delay. At his previous visit six months ago, his weight was 10.6 kilograms, and it is now 11.8 kilograms, indicating a weight gain of 1.2 kilograms. His growth is following the 50th percentile for weight and the 98th percentile for height. There are no concerns with growth or weight loss. Ethan’s diet primarily consists of bread, small amounts of fruit, and milk, with limited intake of other food groups such as vegetables, meat, or eggs. He consumes approximately four ounces of milk multiple times a day and has a preference for bread and cookies. His language development is delayed, with a vocabulary of approximately five words and difficulty forming two-word phrases. His siblings did not experience similar language delays at this age. He is not currently attending daycare or a structured program with peers. The family speaks Urdu at home. REVIEW OF SYSTEMS: CNS: Grossly normal. RESP: Clear bilaterally, no wheezing or crackles. CVS: Normal S1 and S2, no murmurs, good capillary refill. GI: Soft, non-tender abdomen, no hepatosplenomegaly, no masses. DERM: No rash or skin abnormalities. MSK: Normal range of motion, no joint swelling. IMMUNIZATION: Not assessed. ALLERGIES: No allergies reported. ACTIVE MEDICATIONS: None reported. DEVELOPMENT: Ethan exhibits delayed language development. He does not speak in two-word phrases and has a vocabulary of approximately five words. He responds to his name, maintains good eye contact, and understands instructions. SOCIAL HISTORY: Ethan lives at home with both parents and two older siblings, aged ten and nine years, who are currently attending school. The family speaks Urdu at home. He does not attend daycare, and there is no interest in sending him to daycare. PHYSICAL EXAMINATION: WT: 11.8 kg (50th percentile) HT: Not assessed (98th percentile for previous height measurement) BP: Not assessed HR: Not assessed HEENT: No abnormalities noted. Chest: Clear bilaterally, no wheezing or crackles. CVS: Normal S1 and S2, no murmurs, good capillary refill. Abdomen: Soft, non-tender, no hepatosplenomegaly, no masses. MSK: Normal range of motion, no joint swelling. Derm: No rash or skin abnormalities. CNS: Grossly normal. DIAGNOSTIC STUDIES: Blood work was recommended to assess nutritional status and rule out any underlying deficiencies. IMPRESSION & PLAN: Ethan has dietary concerns and language delay. His weight is appropriate for his age, but his diet lacks variety and nutritional balance. He has a limited vocabulary of approximately five words and does not form two-word phrases. 1. Dietary Concerns: - Advised parents to offer a variety of healthy foods, including vegetables, meat, and eggs, in small portions during meals. - Recommended withholding alternative foods if the offered healthy options are not consumed, to encourage proper eating habits. - Discussed the importance of respecting hunger cues and avoiding frequent feeding outside of structured meals. - Suggested parenting classes, specifically the Triple P Positive Parenting Program, to address behavioral and feeding discipline. 2. Language Delay: - Referred to an early intervention program for speech and language therapy. - Recommended participation in a parent-and-toddler program at a local school to encourage interaction with peers and language development. - Advised follow-up in two months to assess progress in language development and feeding behavior. 3. Behavioral Concerns: - Discussed the importance of consistent discipline and setting boundaries to manage behavior. - Recommended parenting strategies to establish authority and improve behavioral outcomes. - Suggested follow-up in two months to evaluate behavioral improvements.
