HISTORY OF PRESENT ILLNESS The patient presents for follow-up of neurological symptoms following completion of chemotherapy. The patient reports recurrence of prior symptoms, including fatigue, numbness in the fingers, and unsteadiness during ambulation. The patient was evaluated by a neurologist last week, who recommended further diagnostic evaluation with magnetic resonance imaging and nerve conduction studies to exclude new pathology. The neurologist indicated that the current symptoms may represent residual effects of prior oncologic treatment. The patient denies any new neurological deficits beyond those described. There is no report of new or worsening pain, seizures, or visual disturbances. The patient expresses concern regarding possible disease progression but is reassured by the absence of new lesions or tumor recurrence on recent imaging. The patient’s daily activities are impacted by fatigue and gait instability. The patient has not experienced adverse effects from recent oncologic therapies beyond the described neurological symptoms. The patient has not been hospitalized since the last visit. Recent MRI does not show any new lesions or tumor recurrence. There is post-treatment inflammation near the spinal cord, which may explain the nerve-related symptoms. Laboratory results indicate that the patient’s blood counts are recovering post-chemotherapy, though inflammatory markers are mildly elevated. Currently, the patient continues to experience fatigue, numbness in the fingers, and unsteadiness during ambulation. Denies new neurological deficits, pain, seizures, or visual disturbances. PAST MEDICAL HISTORY History of chemotherapy for oncologic treatment. No history of excessive bleeding, bruising, or thromboembolic events. No recent transfusions. No mention of family history of anemia or malignancy. No history of autoimmune disease or adverse pregnancy outcomes. No hospitalizations since last visit. ALLERGIES TO DRUGS Not available. MEDICATIONS Not available. SOCIAL HISTORY FAMILY HISTORY PHYSICAL EXAMINATION PLAN Patient with history of chemotherapy for oncologic treatment, presenting for follow-up of neurological symptoms post-treatment. I had a discussion today with the patient regarding the recurrence of neurological symptoms following chemotherapy. We reviewed the recent MRI and laboratory results, which do not show any new lesions or tumor recurrence, but do indicate post-treatment inflammation near the spinal cord and mildly elevated inflammatory markers. The patient has been referred to a physiotherapist with expertise in oncology to address issues related to balance and mobility. Neurological symptoms will be monitored closely, and if there is any worsening of these symptoms, repeat imaging will be considered in six weeks. The patient has been instructed to maintain a symptom journal to help identify any patterns or changes over time. At this stage, no further chemotherapy or radiation therapy is required.
