Subjective:
Chief Complaint:
White spots on tongue and throat

History of Present Illness:
Priya Sharma, a 38-year-old female, presents with a chief complaint of white spots on her tongue and throat. The patient reports noticing these spots approximately one month ago. She initially consulted her dentist, who did not find any significant abnormalities. Subsequently, she was evaluated by Dr. Patel, who performed a throat swab that did not grow any bacteria. Dr. Patel prescribed a course of fluconazole, nystatin, and oral antibiotics, but the symptoms persisted despite treatment. The patient denies any associated pain, changes in breath, or presence of blood in saliva. She has no history of smoking and reports only occasional use of betel nuts, approximately five to ten times in her life. Her dental hygiene is good, and she brushes her teeth regularly. The patient has no significant past medical history related to this complaint. She was referred for further evaluation due to the persistence of the white lesions despite prior treatment.

Past Medical History:
The patient previously consulted a dentist who did not identify significant abnormalities. Dr. Patel performed a throat swab which did not grow any bacteria. The patient was prescribed fluconazole, nystatin, and oral antibiotics, but the white lesions persisted. The patient was referred for further evaluation due to the persistence of oral white lesions despite prior treatment. The patient missed a specialist appointment with Dr. Morgan and was informed of responsibility for charges related to a no-show if it was her fault.

Social History:
No history of smoking. No regular betel nut chewing, only occasional use.

The Review of Systems:
No pain in the mouth
No change to breath noticed
No blood seen in saliva
Teeth are in good health
No hygiene issue with the white tongue

Current Medications:
Fluconazole, nystatin, and oral antibiotics were previously prescribed.

Objective:
Physical Examination:
Mouth:
- Lips: normal color
- Central cyanosis: absent
- Oral mucosa: white line on buccal mucosa consistent with chronic irritation from teeth, no concerning lesions
- Tongue: white coating present, no loss of papillae, symmetrical
- Teeth and gingivae: normal
- Posterior pharyngeal wall: normal
- Tonsils: normal, no white spots or exudate observed
- Soft palate motion: normal
- Retromolar trigone: normal
- Stensen’s (parotid) duct orifice: not assessed
- Wharton’s duct orifice: not assessed

Test Results:
Throat swab did not grow any bacteria.

Assessment:
Problem:
Persistent white lesions on the tongue and buccal mucosa despite prior courses of antifungal and antibiotic therapy. No associated pain, halitosis, or bleeding. No evidence of acute infection or malignancy on examination.

Differential Diagnosis:
1. Oral leukoplakia: She presents with persistent white lesions on the tongue and buccal mucosa, unresponsive to antifungal and antibiotic therapy, and with no associated pain or bleeding.
2. Benign frictional keratosis: The white line on the buccal mucosa is consistent with chronic irritation from teeth.
3. Oral candidiasis: Considered due to white coating on the tongue, though prior antifungal therapy was ineffective and there is no acute inflammation.
4. Nothing concerning for cancer: No suspicious lesions or features suggestive of malignancy are identified on examination.

Plan:

• Leukoplakia, not yet determined (K13.21): White lesions observed on the tongue and buccal mucosa, with no associated pain, halitosis, or bleeding.
• Re-swab of the oral cavity to assess for yeast colonization
• Advised brushing the tongue with a soft bristle toothbrush and baking soda
• Recommended consideration of probiotic lozenges or probiotic gum that dissolve in the mouth to promote healthy oral flora
• Advised to avoid unnecessary antibiotic use, as antibiotics may contribute to oral mucosal changes
• No hygiene issue with the white tongue
• Nothing that needs biopsying
