Bisphosphonates are associated with a low risk of ONJ and AFF, which increases significantly with duration of use, especially amongst patients of Asian descent. Denosumab is also associated with ONJ and AFF, though the risk does not increase with duration of use. However, denosumab is associated with discontinuation rebound fractures and should not be abruptly stopped without transitioning to a bisphosphonate.
