Pediatric Allergy Follow-up in person

Follow-up in person:

Lucas presents for follow-up regarding attention and focus concerns; medication monitoring.

Lucas has been on a 40mg medication regimen, which he reports has improved his ability to focus, particularly in science. He continues to find it difficult to focus in math, which he describes as boring. He enjoys physical education and basketball, reporting increased confidence during sports. His parents note that he has been more tired recently and has difficulty falling asleep despite going to bed by 9:30 PM. Appetite is decreased, with minimal intake in the morning and incomplete lunch consumption. No other acute complaints are reported.

Problems:  1.  Attention-deficit hyperactivity disorder, predominantly inattentive type.
- Lucas has a documented history of attention and focus difficulties in the classroom, with improvement noted on current medication. He continues to experience some difficulty focusing in math, but reports increased confidence and focus during sports.
- Side effects of medication include increased fatigue, difficulty initiating sleep at night, and decreased appetite, particularly in the morning and at lunch.

Physical examination:
Height is in the 85th percentile for age, and weight is average.

Impression and recommendations:  
1. Continue current medication regimen at 40mg.
2. Monitor sleep patterns, appetite, and school performance.
3. Encourage use of a visual checklist for morning routines to promote independence.
4. Advise limiting sugary drinks and consider switching to no-sugar sparkling water.
5. Reassess in the summer for follow-up.
