Objective:

Cold Sore

ASSESSMENT:
Ms. Parker reports persistent fatigue for approximately three months, accompanied by weight gain, irregular menstrual cycles, morning joint stiffness, hair thinning, constipation, and increased sensitivity to cold. There is a family history of autoimmune thyroid disease (mother with Hashimoto’s thyroiditis). Laboratory investigations reveal elevated thyroid stimulating hormone at 7.8, low-normal free thyroxine, and positive anti-thyroid peroxidase antibodies, consistent with autoimmune hypothyroidism. Mild vitamin D deficiency is also noted. No physical lesions or findings consistent with herpes labialis are observed or reported.

RECOMMENDATIONS:
- Ms. Parker is advised to avoid touching the area to prevent spread to other sites, keep the lesions clean, and wash her hands frequently. Hands should always be washed after application of topical treatment. She should avoid kissing or sharing utensils/straws/drinking glasses with other people until lesions are fully healed.
- Treatment: Document details of prescribed treatment including dosage, frequency, duration of treatment, and instructions.

MONITORING/FOLLOW UP:
- Follow up in 3-7 days to assess response to treatment.
- Ms. Parker is advised to contact their primary care provider if their symptoms do not improve within 14 days or there are signs of bacterial infection.
