HPI:
The patient is being followed for chronic heart failure and hypertension. The patient reports that blood pressure readings at home are low, with a measurement of 90/53 mmHg this morning and a reading of 79/57 mmHg at the dentist yesterday. The patient denies any dizziness associated with these low blood pressure readings. The patient reports no complaints about the current medication regimen. The patient denies any new symptoms or concerns.

ROS:
Cardiovascular – No dizziness when blood pressure drops to low levels. Respiratory – No breathing issues or shortness of breath during regular activities. The review of systems is otherwise unremarkable.

Objective:
Vital Signs:
Blood pressure measured at home is 90/53 mmHg this morning. Blood pressure measured at the dentist yesterday is 79/57 mmHg. Other vital signs were not documented during this visit.

Assessment:
