Secondary Survey - trauma template

Patient Information:
The patient presents for an in-person evaluation due to polyarthropathy with severe right hand pain and functional impairment. The patient does not know who the specialist is.

AMPLE History:
- Allergies: No information available
- Medications: Acetaminophen and non-steroidal anti-inflammatory drugs
- Past Medical History: Polyarthropathy, possible psoriatic arthritis under consideration
- Last Meal: No information available
- Events Leading to Injury: The patient reports progressive, severe pain and swelling in multiple joints, most notably the right hand, with associated functional impairment. The patient is staying off alcohol.

Physical Exam:

- Abdomen/Pelvis: 
- Extremities: Marked tenderness and swelling over the metacarpophalangeal joints, especially the fourth digit of the right hand. Similar changes are observed on the left hand. The patient reports pain in the toe, knee, and back. Functional impairment of the right hand is noted.
- Back: The patient reports pain in the back.

Assessment & Plan:
- Summary of Findings: The patient presents with severe polyarthropathy, most pronounced in the right hand with marked swelling and tenderness of the metacarpophalangeal joints, especially the fourth digit. There is associated functional impairment, pain in multiple joints including the left hand, toe, knee, and back. Blood work did not show anything alarming.
- Interventions Performed: Intravenous methylprednisolone administered. Plan to initiate a short course of oral prednisone. Work note provided for occupational support.
- Pending Procedures & Investigations: Awaiting rheumatology specialist evaluation on February 24th.
- Disposition: Outpatient follow-up with rheumatology.
