DATE: [Insert Date] REFERRING PHYSICIAN: [Insert Referring Physician Name] Dear Dr. [Insert Last Name], OPERATIVE NOTE: PROCEDURE: State the procedure performed using concise standard operative wording. Include the main operation and any key related procedure only if explicitly stated, such as biopsy, wide local excision, lymph node biopsy or dissection, laparotomy, laparoscopy, bowel resection, hernia repair, flap, graft, drain placement, stoma creation, or other reconstruction. Do not add procedural elements that were not performed or not clearly described. PRE OPERATIVE DIAGNOSIS: State the diagnosis or operative indication before the procedure as supported by the dictation or surrounding clinical context. Include lesion type, mass, cancer diagnosis, biopsy-proven pathology, suspected diagnosis, postoperative complication, or operative indication only if clearly stated. Do not infer a diagnosis beyond the available clinical context. POST OPERATIVE DIAGNOSIS: State the postoperative diagnosis if explicitly stated. If the postoperative diagnosis is not stated but there is no indication that it changed intraoperatively, use the same diagnosis as the PRE OPERATIVE DIAGNOSIS. If the intraoperative findings clearly changed or refined the diagnosis and this is explicitly stated, reflect that accurately. Do not invent a new postoperative diagnosis. SURGEON: [List the operating surgeon only if explicitly stated] ASSISTS: [List assistants only if explicitly stated, including residents, fellows, physician assistants, nurses assisting, or medical students if provided] ANESTHESIA: Briefly document the anesthesia used if explicitly stated, such as general anesthesia, local anesthesia, regional block, MAC, sedation, or local with sedation. Include specific anesthetic agents, concentration, or volume only if explicitly stated and relevant. Do not infer drug, dose, route, or anesthetic type if not provided. PROCEDURE NOTE: Document the operation actually performed in a concise but clinically complete manner based on the dictation. Begin with the patient being brought to the operating room or procedure setting only if stated or clearly part of the dictated operative flow. Include consent, positioning, prep and drape, antibiotics, time-out, or site marking only if stated. Preserve the chronological steps of the operation as dictated. The narrative should follow what happened in sequence, including incision or access, dissection, exposure, identification of relevant anatomy, resection or biopsy, specimen handling, hemostasis, reconstruction or closure, dressing, drains, and patient disposition if stated. Include clinically relevant details when provided, such as: - Operative site, side, lesion, mass, nodal basin, abdominal quadrant, breast location, skin/soft tissue site, or organ involved. - Lesion size, excision dimensions, margins, depth, planes of dissection, or anatomic relationships if stated. - Key intraoperative findings, including tumour appearance, fixation, invasion, adhesions, inflammation, hematoma, seroma, infection, necrosis, lymphadenopathy, peritoneal findings, bowel findings, or metastatic disease if described. - Important structures identified, protected, divided, or preserved if specifically mentioned. - Specimens removed and how they were oriented or submitted if stated. - Reconstruction, anastomosis, flap, graft, drain, stoma, mesh, dressing, or closure details if stated. - Estimated blood loss, complications, sponge/instrument counts, patient tolerance, or transfer destination only if provided. If specimens were sent, include what was sent and for what purpose when stated, such as pathology, frozen section, cytology, microbiology, or culture. This can be integrated into the narrative or stated near the end of the PROCEDURE NOTE. If there were complications, include them only if explicitly described. Do not state “no complications” unless the dictation explicitly says this. Do not fabricate routine operative steps, normal findings, counts, EBL, drains, dressings, or postoperative disposition that were not stated. POST OPERATIVE FINDINGS: Briefly state the key operative findings and immediate postoperative result if explicitly described. This may include lesion excised, mass biopsied, margins grossly clear if stated, nodal basin findings, intra-abdominal findings, important structures preserved, absence or presence of gross metastatic disease, wound appearance, drain placement, or patient tolerance. Do not invent findings or pathologic conclusions not available at the time of operation. If findings are already clearly documented in the PROCEDURE NOTE, keep this section concise and avoid unnecessary repetition. Next appointment: [Insert follow-up date or instructions] Note: The patient consented to the use of AI-enabled documentation software for this encounter. This note has been reviewed for accuracy. Yours sincerely, [physician name]
