**UROLOGIC DIAGNOSES:**

**HISTORY OF PRESENT ILLNESS:**

The patient reports persistent fatigue over the past three months, significantly impacting daily activities. Associated symptoms include weight gain, irregular menstrual cycles, morning joint stiffness, hair thinning, constipation, and feeling unusually cold.

Urologic review of systems otherwise negative. No history of lower urinary tract symptoms at baseline, urinary tract infections, incontinence, gross hematuria, kidney stones.

**MEDICAL HISTORY:**

**Past Medical History:** Healthy

**Past Surgical History:** None

**Medications:** None

**Allergies:** No known drug allergies.

**Family History:** The patient's mother has a history of Hashimoto’s thyroiditis.

**INVESTIGATIONS:**

No recent relevant investigations available for review

I have arranged for a follow appointment.
